Apparently this is Cicero talking about pains in the service of pleasure (for example, work for relaxation and vacations), and pleasures that bring pains with them. Really, pain and pleasure do tend to go hand-in-hand, as Socrates says at the beginning of the Phaedo when rubbing his leg when released from its bonds.
